I need an android application which gets the unique identifier from the mobile phone (IMSI) to allow the user to answer a survey. The survey will contain only 1 question with 5 multiple options so the application is really really simple. We require a very good design and the results stored in a database we can access (MySql is fine). We also need to store the location in the database. (The user must allow this). Obviously it must be responsive.
