Modify Logo into a Variety of File Formats

I am seeking a graphic designer to transform our existing logo to reflect our new name. Also we need a variety of file formats.
**Changes Needed:**
- Typography: The new logo must remove the "&" from the name. So the new name of the logo should be "Lucidity Sleep Psychiatry." The font style should remain the same. The image part of the logo (not the words) can remain the same.
- Color: We'd like the color to remain black. We would like an additional logo file in white.
-Backgrounds: We'd like separate logo files so the image is placed over 1.) a transparent background, and 2) a white background.
- Proportions: Please size the logo to ensure it looks great on on our website. We'd also like a smaller logo appropriate for a favicon.
**Required Final Formats:**
- JPEG: For general use across digital platforms.
- PNG: With a transparent background for versatility in use over different backdrops.
- SVG: To ensure the logo can be scaled without losing quality, for use in larger formats.
