This needs to be an Australian freelancer. The deliverable I need is a consent form for minors to appear in a video which will be used as a cinema advertisement. It should ensure that once a legal guardian signs the document, both myself (my business) and my client will be covered for the use of this footage. It would be useful if this document could be used for multiple projects in the future, but being able to edit the project name and usage. I am ideally looking for someone who can provide advice on minors in the media to advise on what we can and can't do as well as provide a water tight document that will protect both myself and my client. It would be great if you have provided advice in this field before
