Store brochure template - rewards program

I'm looking for a brochure template to promote a store rewards program. Brochure is double-sided:
Front: Instructions to customer how to participate (involves downloading the loyalty app, entering a code)
Back: Details on available rewards (photos of reward products, how many points customer needs to accumulate, terms and conditions).
The design is intended as a template, so that in future I can edit details myself:
- reward details, photos
- business name
Attached samples:
- current company design style
- basic idea for what front / back could look like, but very open to improvements on that
Final text to use (plus high res images of company logo, icons, photos, etc) will be provided. No logo / icon design is needed.
