Create a simple online assessment tool - Great project opportunity for talented IT intern/recent graduates

Project: Create simple online assessment tool We would like to hire a talented programmer/WEB design whiz to quickly convert a paper-based assessment questionnaire into a simple online questionnaire/assessment tool. The questionnaire is about 2-3 pages. The way it works is the user would complete the questionnaire online and based on the responses, the online tool would tabulate scores and spit out scores/report instantly or close to instant. We expect the project to take less than 2 to 3 duration days and a few hours actual work. You must have intermediate programming and web design skills to be able to design an aesthetically pleasing online questionnaire without supervision. 100% content provided. You must be able to integrate the online tool with PDF or Word. Example: the output must be printable in PDF or Word after the user completes the online questionnaire.
