Menu Design Required - repost

I have Purchased a template for the tri-fold menu design but is in ADOBE INDESIGN. As you can see from [login to view URL]
I need
1: Using the template, apply my menu items and graphics into it and save as PSD and export as PDF and JPEG as finishing.
2: Using the PSD file and create a similar menu WITHOUT pricing/logo and instead of number, put "Please contact staff for ordering"
3: Using the PSD, create a A4 size menu (design can be something like the trifold template provided) and place all menu and items into it utilizing both side of the A4 displaying vertically.
4: Using the A4 PSD create a menu without Pricing.
All graphics for the food item are provided (however i can use your version if you have better source)
Template is in .indd and idml with a folder of links images inside.
Design is required, this is not a cut and paste job. The template is the framework or guideline. The A4 Vertical menu design work is required. Icons like: famous! hot! new! ...etc may be required.
Please do not just bid with I can do it or post me with chunk of profile. Only post me with relevant menu design and some wordings/comments from you what you understand and if any part you foresee there will be questions/problem.
