Data entry from PDF to CSV file

I need to create a product catalogue for an online shop, taking data from the Uhlmann catalogue PDF and entering it into the Allstar Uhlmann products CSV. Key requirements are:
- adding a new row for each size of each product
- adding every item from the PDF document, including all sizing information
- that I am able to take the delivered CSV and upload it to the website
- images are entered in the form of a URL, which I have already uploaded en masse to the sites uploads folder
You will not be required to:
- add pricing information as it is not yet available
- provide a product description as it is not yet available
