Hello we need an electronic waiver form designed and implemented (we have the content) I will need to: 1 Be able to take data from fields already in our website. I.e name, address email, phone, etc and enter them into the waiver form automatically. 2 it will need to have checkboxes on the way to acknowledge that they have read certain parts of the waiver (see attached) 3 They will need to sign at the end with our signature pad 4 it will also prompt to take a photo with our computer camera 5 after they sign and take photo we will have to store either on or server or google drive, dropbox or alike

If so, the steps I'd need to take in order to understand your requirements and deliver on this project would be: 1. Find out what back-end you are using and where the required fields exist in your database 2. Discuss what you mean by 'signature pad', and 3.
