Format my eBook for kindle and insert my name in cover page

I have about a 34 page book in Word that I would like my book cover to have my name on it, and it formatted to professional standards so that I can sell it on Amazon as kindle book, an e-book and print book (eg. on Createspace) as self-published. It need acknowledgements, dedication page, table of contents etc.
For the book cover I require my name on it: Ed Hanada, M.D. and if you need more information eg. dedication etc. I can provide this no problem! I would prefer really formatting in professional manner eg. looks like a real book.
Put my name in easily readable text that is in consistent font on the ebook cover.
