Need an SEO optimized, Secure Medical website to provide Second Opinion advise to patients globally. This project will have multiple phases but this is for phase 1 with following requirements: 1. Website needs to be HTML5 compatible so it can be viewed on all browsers including mobile devices 2. Navigation, forms and layout should be very intuitive and user-friendly. 2a. Easy content management by Admin 3. Secure (HTTPS) Patient portal to sign up and log in 4. Patient information page to enter Age, Sex, Diagnosis and Medical History 5. Ability for patient to upload multiple files (Pathology report, Imaging studies, etc.). Files should be securely stored 6. Patient can request a second opinion by paying the applicable fee by entering credit card information (Ability to accept international) credit cards 7. Notification (email) is sent to Doctor to review case 8. Section for patient to review submitted requests and fees 9. Section in patient portal to review Doctor's notes 10. Secure (HTTPS) Doctor Portal 11. Section to review pending cases in Doc portal 12. Ability to submit notes for a case in Doc portal 13. Requires secure SSL 14. MS SQL or mySql Database backend 15. Patient and doctor's passwords should be encrypted 16. All submitted cases will be assigned a case# so they can be reviewed by Doctor, Admin and appropriate patient Additional pages accessible from Main Navigation: 1. Patient Login/Sign-up 2.
