Convert a Template to a Website

We have the design and look of the website. The website has to be html5 responsive website. (NO WORDPRESS) There are specification and samples of how we would like it to look on internet. We have the PDF ready to be look out on Illustrator, etc. Also, if you need the file we can provide it in any other format. We will provide TXT, Fonts, icons and images on formats jpg, png, gif, etc. Also there will be a pop up window where the customer has to include his/her email address and a check out box in order to go to another page. One electronic form where we need to get our customers, name, email, address, phone, etc. Two Similar electronic forms showing up some numerical sample. (there will be just a simple number change) We will provide a excel sheet in order to see how needs to work on internet. A similar sample will be something like [login to view URL] you fill up the blanks and you have a price. in this case we will only are gonna need 1 field to fill up. (Skype meeting in order to talk about it) For the menu bar. We requiere to be top fix with a blur effect on the background. Please check the files in order to see what we want to achieve. We will like to have ours white background and then blur transparent effect background instead of white background. We can talk more about it as soon as you are selected. Also, there will be rollover effect and color selection on the menu. We will provide the html code color 4 simple CSS3 transition on the home page. 16 parallax effects that will be the same on all of them. We will provide a sample as soon as you are selected. 5 side bar parallax effect that effect will be the green leaves (elements that are on the attachment) when you scroll down on a section of the page the image appears on both side of the screen, left side of the screen will be going up and right side of the screen will be going down.
