Basic Java script

Please view my basic page here [login to view URL] It is a very basic form. Users will select one of the 5 checkboxes in the upper section which will calculate a total int eh "Total" field. Users will also select one of the three radio buttons or the Culino option. The selection of either of the radio buttons will not affect the total. Selecting the Culino option will update the total. On Submit, the form will be submitted with information such as which check boxes are checked as well as total amount, name address etc...
