Change a Web Theme to match PSD

I own a theme called Graphista which is based off of Bootstrap and can be found at wrapbootstrap.com. I will provide you with the entire theme package complete with help files. There is already a domain and hosting setup and I can provide ftp details.
I need someone to modify it to fit my specifications. I have a Photoshop mock-up of the changes needed complete with assets and copy.
Here's the site breakdown and the things needed based off the live preview at wrapbootstrap. You can also view the jpgs attached.
1. Change titles and meta information - copy will be provided
2. Change logo - Logo changes from white to color as nav bar pulls down - this is not supported in the theme.
3. Add social network buttons above nav text
4. Change background image and landing copy
5. Change icons and copy under 'my services' to match psd
6. Change 'I am Jane Doe' to new image and copy that will be provided. BG image will be used.
7. Remove entire 'I got the skills' section - replace with 'Equipment' List.
8. My works Gallery - Change background. I would like to link the images displayed to either flickr or instagram so that the client can easily update images with out modifying html.
9. Link twitter feed to client's twitter account.
10. add working form called 'book now' that will email client
11. minor changes to Contact Me form, make it email client
12. Change footer to match psd mockup.
There will be several small changes and edits before the site goes live.
That's it, with the exception of the gallery and forms everything else is pretty basic simple html and most of the hard work is done by the template.
Please be familiar with Bootstrap and able to work within current bootstrap markup.
