Build an ecommerce site with workload management interfaces

This is the funcationality
1. client creates a personal record
2. client completes a form and uploads a video in the form and pays with paypal
3. the form and the upload video are stored in the database and an email notification is generated
4. A provider profile is in the database
5. The client form is assigned to a provider (this can be manual but should eventually be automated with basic conditional logic)
6. The provider form is pre populated with some information from the client form, including the video or a link to the video, but there is some conditional logic based on the answers that the client provides, which results in additional fields being shown to the provider through the pre population
7. The provider completes the blank field in the populated form and submits the form
8. The submission is saved in the both the provider and the client profiles in the database and an email notification is generated
The interfaces should be easy and a clean look and feel like [url removed, login to view]
