Top Navigation Bar and Form Repair

I need a navigation bar developed with a transparent drop down menu. I need to be able to update the links site-wide from a single file. The navigation bar needs to be installed upon completion. The site is currently under construction. I will send a link to the site to serious inquiries. I also have a contact form that needs repair. The form is already built the code just needs to be fixed to make it work. The instructions for the navigation bar are attached. This is a small project.
