Convert Spreadsheet to HTML Form

Need to convert an Excel spreadsheet to HTML. Form is attached to this project. The only modification is to make the form vertical vs. the current horizontal layout. Attached are two files. The first is "dreamwatts payback [login to view URL] is the current estimator with working cells. The second "estimator-mockup.xls" is how I would like it to look. The resulting form will be placed in a directory on our website and operated through an Iframe within our joomla website. Payment will be made when the form is live online and properly calculating results. This is the first of a series to be created.
