Contact Us webform/ template with captcha & required fields needed

Hi all, I require a webform template for a contact us page. I need to be able to integrate captcha images (already have a passcode etc) and required fields. My preference is also to be able to hide my destination email address to remove the possibility of spam and send an auto response email to verify that the contact request has been received. I would like this template to be easily customisable so that I can change it for different contact pages throughout my site. I don’t require an entire page design, just the code for the form.
