Contact Form and Backend Quote Manager

There are two parts of this project and a example of the quote form can be found here [login to view URL] - it's just a example. I will need captcha and a few extra fields. Part 1 - I need a quote form made that has email validation, required forms, and recaptcha functions. The form will have the following fields: Name, Email, Phone, Company, Website, Photo / Mockup Upload (this is picture format only and needs to have a 200k max), an options to upload 2 more pictures and when clicked it shows an extra field, and a Details field (message field with 400 character limit). The form will send a email to me with quote form information and a link to my Quote Admin Panel. Part 2 - I will need a backend to manage these quotes. I want them nicely formatted with rollovers (not plain text). Each quote in the backend will have to give me the option to mark it as "Contacted, Interested, or Not Interested". When I click on the quote I would like the see their contact information nicely layed out with any images they uploaded. I always have projects and work and prefer to have a good coder who is smart and can find solutions and gives me a good price. So if I pick you, I most likely will keep you around for more projects.
