Design a new form

Select an input or output document that you consider to be poorly designed. Cite reasons why the form is poorly designed. List the modifications you make to improve the forms designed Explain the reasons for your modifications. Re- design the form, applying the changes you sited. Use a software package of your choice to create a new form. Include the re-designed, poorly designed form as part of project. Person must have Acrobat Reader so I can print the forms and work. ## Deliverables Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
