Auto Insurance Quotation Website & Database Design

We are looking for a developer/designer to develop our website and MySQL backend. The website should be developed in web 2.0 style design using Wordpress CMS. The design should be sleek, clean, and the focus should be on directing the user to complete a form to receive car insurance quotations. The site must also include a basic page of SEO. The purpose of the site is to generate auto insurance leads. The site will only be a few pages. As mentioned, the focus on the main page will for prospects to enter their postal code and then be directed to a series of forms where they will enter information for each driver and vehicle on their insurance policy. The forms should be connected to a MySQL database. Once the forms are completed, an automated email should be sent to the user and notification should be generated to an internal account for immediate follow-up (manually by phone or email... no automatic quotation system required) .
