Create Performance Evaluation form in MS Access

I am at loss and need help, like in a weeks time. I need to create a simple performance evaluation form in an small database. I need to have this by the end of this month. The form should be user-friendly and simple to use. The layout is attached below.
Note: There are other criteria that needs to be in this [multi-user][1], single form. I will provide the winning bid with the details. There are also points values assigned to the Yes/No/NA criteria choices. If tables and form can be built to capture the results, queries can be built later on for reporting.
I know there's a lot of wizards/masters who can help me with this scenario before. I'm willing to give an extra bonus if it can be done in les than a week. Besides, I'm trying to impress my "hot" manager.

## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).

The OS is Windows XP using MS Access 2002 but the database can be in Access 2000.
