create a Web form - sign up page for seminar on my website

My website is running wordpress - Nayma theme. I need to make a seminar signup page, where people will sign up (without paying) and enter their name, address, email, phone numbers, and comments. I would need to export the signup list to excel and save it anytime . I need the ability to also close it if the class is full.
Also, would need to re-use the form if we have another seminar in the future. it must match existing theme. our website is [url removed, login to view]

You can write it in regular PHP or use a wordpress plugin.
I will need the ability to CONFIRM the registrant (once they pay us - I will login as admin and mark them as confirmed) or keep them PENDING if they have not paid yet.
