Custom Website Database

I will do my best to describe What I need. I would like a database where login is used with VBulletin to enter in information Website design for database will be minimal. All I want is the Vbulletin Custom Header or Wordpress one. Also I would like a Side bar for Adsense. I have a logo etc. This is an Add on. I also want users of the databse the ability to report something does not work. The Database will have many drop downs, and text fields for the user to fill in. The Database is similar to a Price book. Also if possible I would love to use icash to give away currency for members to input information. This database would be fine to be on a seperate folder or subdomain as long as the vbulletin login is used to add information.
