Database to Catch email data from online contact form

I have a one page contest site with a contact form. Visitors will fill out form (to enter the contest). I need the form contents from each submit to be entered into a database table. Also have restriction set for only 1 email address (No double entry's) and 1 street address (no double entry's). I have the database but no tables yet. I would like the table to be named 'contestants'. It will need 8 rows. first_name,last_name,address1,address2,city,state,zip_code,email.
