Acrobat Fill In Form

I have a 4 page PDF form that needs to be converted to an Acrobat form that may be sent viia email for completion by the recipient. The form would allow entry of values for the form. The form should retain values and be capable of being accessed multiple times locally by the recipeint to change their entries. The form would then be emailed back to me with the recipient entries included in the form. Prefer work be done in Acrobat 8 or 9. ## Deliverables See accompanying file to view the Word and Acrobat document to be changed to an Acrobat form.
