Cloud Portal with mySQL tables and written in PHP

I need a simple portal for MySQL database and programmed in PHP. Must be completed quickly. Data base has three tables: contacts, vehicles, and alerts. Need code to create, modify, and delete contact & vehicle records and an update screen to change the status of the Alert table records. Tables have less than 10 fields per table.
