"Printable Chore Chart" Script for Website

I need a simple custom javascript application for my website. My website is about Chores for Kids. My server is an Apache server that is running Wordpress in a standard configuration. I don't need a Wordpress "plugin" developed, but I do need the ability to run the script within the wordpress environment. The script needs to be easy-to-run, with minimal "clicks" required of the user and output a aesthetically pleasing chart. The goal of the script is to create a "chore chart" that parents can print off to use with their kids. Here are the requirements: 1) Input child's name 2) Select a gender. This selection would change the output of the script. So "male" would make a boy themed output (blue) and a "female" would make a girl themed output (pink) 3) The user should be able to select from a list of chores (which I will provide) or 4) Add their own custom chores The output would be a PDF which can be easily printed from the user's browser. I have attached a screenshot taken from a Google image search for the term "printable chore chart". I don't own the rights to this image. I have no idea who owns the copyright to that image and don't want to look exactly like that (I'd like to be able to add the name, specify gender etc.) But the attached image is generally what i'm going for. I know that I am a little loose with some details (boy theme, girl theme) and exactly the steps of the output, but I am hoping to find a developer that will be able to add something to the project and understands what I'm going for here.
