Design A Custom Wordpress Template - Very Basic

Hello - I need someone to design a very basic wordpress template for me. I have attached 2 files of what I would need/want the template to look like.
Basically it would be a very fast loading and basic wordpress template with a few adsense ad slots. It would have a banner at the top, a 728x15 adsense block, with the wordpress page menu under it in plain text (see prototype site attached). The rest of the blog would be very simple with a image adsense block in the upper left of the article area with the text wrapping around the ad, and a normal widget on the right hand side of the theme.
Please, ask any questions before bidding - if you bid without reading the requirements I will immediately reject your bid and report you to freelancer for wasting my time. All bids should begin with "I have read your bid requirements".
This is a very basic theme which should be able to be completed in the $50 dollar or so range.
I will pay milestone payments of - 10% upon awarding of the contract, 25% when first draft is received, 25% when 2nd draft is complete, and 40% upon delivery of completed theme.
Please do not bid if you do not understand - ask first. If you are a good designer you should be able to do this in less than 1 hour .
