I have experience in creating and customizing various survey taking tools. Google Forms does not give much flexibility to design surveys as you want, maybe we can discuss and decide on a better tool which suits your requirements.

Hi, by contextual survey I assume you mean a branching tree system in which certain options bring up certain questions. This bid is based on 3 components: survey viewer with data structure ($300), GUI interface for creating surveys ($200), and the entry of questions and their locations on the tree ($25/hour). You mentioned currently using Google Forms, so send me a PM with the functionality you want added and I'll look into its viability with the Forms system.
