Contact Form with datas kept in database and .csv download(repost)

Hello all, I would need a contact form in PHP that saves the datas received in the database. Each time somebody fill in the contact form, an email of information is sent to the specified address. Contact form should contain some classical information field : Title Name First Name Address 1 Address 2 Zip-Code City Country (Drop down with UN official list can be provided) Email Tel Fax Comments Radio button for Disclosure (yes/no) (text to follow) Customize Check box/Radio buttons to add some choices for the person filling in the form for example : Do you want to receive info about : - Congress - Registration - Sponsorship The "congress/registration/sponshorship" is added by the admin. Database download in csv should be included in current CMS. Please let me know if you have any question.
