Develop simple iPhone application 2

We want a very simple application which is a series of screens where tehsuer selects 1 option from a list per screen , then calls a MYSQL stored procedure database with details of the selection. The stored procedure will then send back a list of selections for the next page. At the end of all the selections theta needs to be saved to a MYSQL database. The application will not require a login screen but does need to let the user enter their email address as a Settings option
The idea is to create a very simply wizard for defining the parameters of a Finance product. We will provide a prototype of the app using Proto.IO. We also ned the web service to allow the App to interface with MYSQL developed but will provide the MYSQL database.
We would like the developers to also register the application with the Apple App store
