Add Facebook Comments on my Wordpress Website

I have a wordpress website with a blog where I would like to add facebook comments so when people comment, it uses their facebook information to comment, as well as gives them the option to post that they commented on my blog on their facebook page. We had them installed previously and the person manually put in code for the comments section instead of using a plugin. Which was fine, and worked well but when I updated my wordpress to 4.0 as well as my wordpress ultimatum theme, it must have deleted the code because the facebook comments section is gone and it replaced it with a weird comment section along with some weird code. Also, to be very specific, I want to make sure that there is a separate comment section for EACH blog post. I want every blog post that I create to be a new stream of comments. I would prefer to have this all set up so I don't have to add any code or do anything different to get the separate facebook comment boxes every time I create a new blog post. I attached a picture of another blog and how I would like mine to look like, as well as a picture of what our issue is.
