Card to be put in transparent pocket on front of file
Should have Name of the patient, Age, sex, blood group, contact no.
OP card
A4 size
Must have the logo (already designed) and the tagline (which you have to come up with)
on the footer end of each page - there must be address and contact no. of the hospital
Visiting cards
Visiting cards for doctors and management staff must differ
We are an ENT (ear, nose, throat) hospital, so be creative
It should ideally have the logo, name of person, his contact no., email id, hospital website address and hospital physical address
Book post covers
generic book post cover size
must have logo, physical address and website address
can have specific area in the front to give details about whom the book post is address to
CD/ DVD label print and housing sleeve
we hand out diagnostic and surgery DVDs to patient
we need you to design the sticker to stick onto normal DVDs
also the DVD cover into which DVD goes must be designed
both must have the logo, name of hospital, name of patient, procedure and the date it was performed
ID cards
For staff and doctors
CT scan and xray covers
CT scan covers must be able to house 14 x 17’’ CT scan
X ray covers must be able to house 10 x 12’’ Xray film
Creative will be printed on plastic cardboard
should have logo, name of hospital, name of patient, age, sex and investigation performed
Letter pads
For rough use within hospital premises
Low costing while production must be kept in mind
Pens, bags
