Build online form that populates a word document and emails the doc

We currently have an offline form in a word document that needs to be built into an online form from scratch with validation. Once the form is submitted, it needs to populate a word document with the information entered and then emailed to an email address. The word document that is created from the data entered will need to be in the same layout as the sample attached.
This will need to be done in PHP and use of Jquery validation were needed.
