Convert the a pdf form into excel spread sheet. There are about 1,320 entries on the pdf form. Each on look like this:
Firstname F. Lastname
Chief Job Title
Company name
Address
city, State zip-code
The excel spread sheet must have fields: Name, position, company, address, city, state, zip
