FACEBOOK FAN PAGE USING CSS ---- URGENT

This project is for a Facebook Fan Page and two additional layouts that integrates the video contest widgets provided by Wildfire ([login to view URL]) using CSS including an embed of a video code on the Fan Page. The two additional pages will be a "Entry Form" page and I am looking for something with a look and feel similar to [login to view URL] for the main landing page, also I need a "Invite Friends" page layout, and "Entry Form" layout similar to the attached which I show you in PM. I will provide logos, pics, and a website that you will match the look and feel of.
