Hi, - I have a survey of questions for a user to fill in (tables and radio buttons with checklist) - Each list item needs a photo "proof" of each item in the survey. - Then the user will submit the form, and we want to save and view the document. I would like to create a web-app, or website that would prompt the user to take a picture (or upload from library) that would then attach itself to each line item of the survey. User will need to check certain boxes, make comments on some rows, and take pictures using iphone or android. This would result in a report of all the answers to the questions and have photos attached where applicable. - Could be compatible with existing wordpress site, or stand alone - Could be limited to either iphone or android, though cross platform would be best. - Web-app , not native mobile app for now unless otherwise prefered - Saving document, or exporting to database or document REQUIRED - Viewing the results afterwards also REQUIRED - I have an AWS account as well as website on Bluehost. - I have additional staff that could help with database creation - I would like to see this project take less than a week, and i would like daily and regular updates Please include : "HI ANDY!" into the intro to your your bid response so i know you read this entire description first to not waste any time. ;) Thanks for Reading ! -Andy
