Time sheet form

Our customer needs a form to be placed on a wordpress page. The form is to have the following properties: Column 1 - Day (Selection of days Monday to Sunday) - Maybe a calendar will work Column 2 - Job Reference (A selection box of job references [projects]. The customer would need to be able to update this list in a user friendly way) Column 3 - Start time Column 4 - Finish time Column 5 - Lunch break Column 6 - Description of work (paragraph text field) Column 7 - Machine (A selection box of machines. The customer would need to be able to update this list in a user friendly way) Column 8 - Hours (Total of hours worked - minus break/lunch time. If no lunch/break has been entered then this must automatically deduct 30 minutes (0.5 hours). Additional info: 1. We need to be able to select up to 21 rows. Employees can work on 3 different jobs in one day and therefore there could be 21 entries. I suggest we display 5 rows and have a button to 'Add new row'. 2. There cannot be any over lap on times. For example; a user shouldn;t be able to create an entry for 1 job on a Monday for between 9am and 1pm and create another entry for another job using the same times. 3. The form needs to total up the weeks hours. 4. We need to be able to export the info to Excel and the form needs to be sent to an email address when submitted. 5. It needs to be suited for mobile use as most users will be entering times using mobile phones. Therefore design needs taking into consideration.
