Super simple Google Map script...

Hi,
I need a simple application (preferably written in PHP) & uses google maps API.
-- I will enter multiple addresses (one by one or all together).
-- your app will plot/specify the address on the map.
-- while entering the address, I will specify a radius (in kilometers) for that address.
-- The actual address should be a small map pin icon
while the radius (for that address) should be highlighted in a transparent color.
That's all! :)
So the purpose of this script is to see multiple addresses on the map + a highlighted area surrounding these addresses (the radius area around the address).
Basically I'm looking for a tool similar to this one:
