SQL Database driven website

We require a website built that manages our suppliers. 1-Contains a list of company names, addresses, phone numbers, mobile phone numbers and email addresses. 2-Linked to the companies a list of staff along with their training competencies and dates any training needs to be renewed. Also ability to upload photographs of staff, and copies of their training certificates. 3- Searchable page based on capabilities, location, name and company names. 4-Restricted access by user names to be able to view, delete, update details of companies or their staff and add/remove users. 5-Export to Excel search results. 6-Separate page to create and add training modules 7-Separate page to create and add capabilities that can then be applied to each company or individual staff member. 8-Automated email sent to company when any of their training modules are expiring. 9-Reporting page showing training modules expiring within next 90 days for our internal management. The task requires a website and SQL database is built for us. Once approved all files supplied including necessary files and instructions for us to host it on our own web space running mysql and php.
