Simple HR Leave/Sick tracking website

We have an old Leave/Sick tracking system in Lotus Notes. The software is so old that it only works on older computers. We are going to switch to Kronos in 2-3 years, but need something to replace the current Lotus Notes system as it keeps crashing. Basically, we need a web site (preferred in PHP) linked to a Microsoft SQL Server (that is a requirement at our office) that allows the HR person to enter vacation time and sick time for our employees. We have about 1,700 employees, so we need the accrual to be automatic, either by a monthly cron job or a MS SQL scheduled task. You can design the database how you feel is best. I will have most of the data in a CSV file that will need to be imported to SQL Server. We probably will start with a few records, and then eventually import all ~1700 users into the database before we go live. The website will be for internal use only, it will not be public facing. But we do need a simple login for the HR folks to update data, and keep all other employees out from modifying the data. On the first of every month, each full time employee gets 6.66 hours of sick time and between 8-14 hours of vacation time, depending on their years of service (0-5 years, 6-10 years, 10-15 years and 16+ years). Part time employees get less, depending on their FTE percentage, for example, 1/2 time employees get 3.33 hours of sick time, etc. There is a cap on vacation and sick time for employees. For example, 0-5 year employees earn a max of 192 hours a year. If they have more than 192 hours on July 1, it is lost/capped at 192. On our annual fiscal year (July 1), we need to roll over any excessive sick time over the cap to vacation time, at a rate of 5 sick hours to 1 hour vacation time. For example, if someone has 400 hours of sick time on July 1, we will take 400-360=40 hours then 20% of 40 hours is 8 hours. Roll that 8 hours over to vacation. If vacation is maxed out to the cap, they lose that 8 hours. The HR person needs to be able to manually override sick or vacation time. Maybe someone forgot to turn in their leave slip, or someone took time off without pay, then they need to adjust the leave time manually. We need to print two different kinds of reports, one a summary and one is detailed. I have attached a few pages of them as PDFs to this bid. Please look at them and review. The Leave Summary is more detailed for each user. Notice for some users, the report is put on two pages. We would like to fit it all on one page. The second page is where the HR person's override information is put in, and it is where the roll over or cut off at the beginning of the fiscal year is reported. We would like to print reports either by Org Unit or by name. For example, If we want to print the reports of everyone in DCJ org unit, then the report should show all DCJ org unit. We also have State Patrol, indicated by SPxx where xx is the troop number. We want to be able to print either all patrol troop numbers, or specify a troop, such as SP1A for troop 1A or SP* for all troops. The second report is more of a summary of all employees' leave balance. It is in alphabetic order from A to Z by last name. We would like it to look like the Sick and Annual [login to view URL] - but get the top columns lined up a little better. I will have a Ubuntu 14.04 test server for you to get on with both FTP access and SSH access. I will also give you access to a test SQL server (Microsoft SQL Server 2008 R2 Express). I'm not sure if you will have SQL Server Studio Management Studio access from your remote location or if you need to RDP into the server to use SSMS. I'm still working on it. We will only communicate via emails, Freelancer PMs or Google Hangouts/Chat. There will be no phone, voice or video conversation held.
