reskinning a credit card form php html page with a new theme keeping same functionality

We have a credit card form, loaded in a mixed php/html code with some scripts to auto space the card number 4 digits a part and auto jump between month and year in expiry date, and some code to differentiate between 2 digit year and 4 digit year, and taking form items and posting them to a credit card processing page.
Bidder must be able to take a photoshop master file and slice it into assets that can be used to re-skin the form, but keeping all internal logic intact! the last thing we want at this stage is re-code it.
