I have an extension Amasty Request Quote that works fine with the default template (Luma), but we have a few minor issues with Infortis Ultimo. An override should be made so that the extension matches the Infortis Ultimo layout. We can't provide access to our Magento installation. Development and testing is on your own local installation. Our server is running in Linux Ubuntu 16, PHP7.2, Magento 2.3.1. Please check the document for screenshots.
