I need a webpage for internal orders to our warehouse. I have started the simple layout of the page for the first part of the order (attached), but it needs to dynamically add table/form for parts to be ordered. Parts to be ordered are in two steps. The job line that it is required for and the part to be ordered. For example. Job line: 1 Part required: Oil filter Job line: 2 Part required: Hose Part required: Hose clamp Also be able to remove a job line with its associated parts or a single part from a job line. Once submitted needs to be able to produce MySQL insert statements to input in a database. Preferably one table for the order and one table for the job lines and parts. I can create the tables in our MySQL server.

I can do the needful but only if you are willing to reconsider your budget. Is the form attached, already coded and saving data to database? I option selection etc. on the order form does not impact the parts ordering part? Order number is not being entered so against what do we save jobs and parts? We give ability to add as many joblines against an order and as many parts against a job line? Please share MySql database schema / structure with sample data. Above details and inputs will enable me to assess scope of work precisely. I will revise / confirm my bid accordingly.
