I know how to implement and trace the WHMCS server provisioning module. When you accept order then it calls the Account Create function from server provisioning module. So it might issue on that function or APIs called from that function. I am WHMCS expert and i have been working in WHMCS since 5+ years and in PHP since 12+ years.
