Custom Wordpress Order Form

In need of a custom single-page product order form, preferably based on Wordpress, but standalone PHP/MYSQL will also work.
The order form will have information pre-filled on a back end. The back end should be easy to use.
The pre-filled order form will include an invoice area, so the back end should allow for the inserting of one or several line items to be displayed on the front end of the order form.
Once completed, the order form should generate a receipt in invoice format, to be emailed to the user, as well as to the form admin. If possible, an order page should be created in case the user needs to review the order after placing it (a page which they can bookmark that shows the details of the order they placed with the shipping/billing information and last 4 of credit card.
A second email with only the order information should be emailed to the admin (See order details in the attached image)
An order ID should be assigned to each order
