Creating an online questionnaire with database and contents

Hello,
I need an structured online questionnaire for school starters. I looked in the internet for an example and found a website that I like to have as a 1-to-1 copy (but not with all functions).
This project is to be built in Bootstrap + PHP + MySQL. The 3 steps to realization are:
1) Analysing the example website and creating of a corresponding database structure
2) Building a website (backend to add and manage the catalogs and the questions + frontend for answering the questions)
3) Fill the ready website with contents from original website (about 40 Questions)
Not all functions are needed. The user management, the evaluation with diagrams and PDF output etc is not necessary. The graphic design comes from Bootstrap. Needed are these functions "only":
Backend:
- create a new catalog for a topic
- create new questions to one or more catalogs
- every question has to have
- a free formulatable question
- a free formulatable example answer
- a free formulatable description as help
- every question must be able to be answered with
- a free formulation (text)
- a multiple choice (option)
- a data table, matrix of values (array)
Frontend:
- display questions and answer options
- save different versions (sessions) of answers
