Always Running Taskbar Application in VB.NET

I need a Desktop Application running in Taskbar programmed in VB.NET. The software needs to start when Windows Starts and stay in taskbar. The screen can be pulled up when clicking on the icon or pressing (Windows + C) key combination. Once the screen is pulled up, it will have only three fields 1. First Name - TEXT(50) 2. Last Name - TEXT(50) 3. Contact Information - Text(50) Using this I can save a contact in an access database and once saved. This will show a listview populated with all the contacts in database and pressing the close button will minimize the application to task bar.
