464802 Modifications in Virtuemart

I need to modify the "Ask a question about this product" form to display the product name sku and have the user select the related products that he is interested in using checkboxes. I also need the user to fill in additional information like company name, telephone number, company address and country. Please view attached image for a clearer idea of what I mean. I also need an existing tooltip on my browse page to display the related products instead of what it is displaying now.
