Using Visual Web Developer 2005 (Express Edition or otherwise), AJAX (Including the toolkit and futures CTP), and SQL Server 2005 (Express or ?), create a very simple, intuitive, but very Web [url removed, login to view] survey creator using the above tools. The language can either can eiher be c# or visual basic. I want the pages to use CodeFiles, StyleSheets, MasterPages, Themes, etc. Basically as much of the available features of [url removed, login to view] (or Flash). Most of this stuff is automatically generated by Visual Web Developer, I just want them to be included in the pages. The visual design is not very important; I will change all that later. Just google survey wizard (or create or creator) to see basically what I want. Start with the very basic LoginStatus, Login and CreateUserWizard for membership, then allow the user to be able to add/edit/delete surveys using some of the data controls. Each item should have a placeholder for a context sensitive help, meaning have a section on the right side of the screen that shows the user what the field is when they hover or have the control in focus. ## Deliverables

The survey creator basically consist of these fields: Step 1 (Maybe use the Accordion control for each step) Current Status (Dropdown) Category(ies) (checkboxes, one or more items) Title (Textbox, with a "remaining characters" counter) Description (same as above) Image (File Upload/URL; user should be able to specify either) Next Step (Button) Step 2 Choices (Start with 2 textboxes, have a + symbol at the end of the second textbox to dynamically add another textbox, have + - to add or remove further textboxes) Previous Step (Button) Next Step (Button) Step 3 Survey Type (DropDown - Text Only (nothing, default) Images (File Upload/URL) Video (URL) Audio (URL)) For each choice in step 2, it should dynamically show the same number of items for this step. Previous Step (Button) Next Step (Button) Step 4 Source Name (Textbox) Source URL (Textbox) Same functionality as step 2 after the first textbox, meaning people should be able to add/remove sources. Start Date/Time Close Date/Time Resolve Date/Time Finish (Button) -------------------------- 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):

a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.

b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
